The Surry County Community Health Assessment, or CHA, is an initiative carried out every 4 years to locally determine the state of health and wellbeing in the county. Every local public health agency has to conduct one, and they vary greatly from county to county. The Surry County Health & Nutrition Center is responsible for the CHA here and is composed of a 5-person core team with over 20 partner agencies assisting. What makes the CHA so important is that it sets the health priorities for the entire county for the next 4 years! This influences funding, changes new and existing initiatives, and provides additional literature for science.
A critical part of the Community Health Assessment is learning what our people go through and what impacts their health. View the opportunities below to share your experience and perspective.
When we compile the final report, we want the data to be relevant to you and your community. Certain conditions and problems affecting the City of Mount Airy may not be relevant to people living in Shoals, and vice versa. We want to accurately reflect each individual community as accurately as possible while also keeping the community recognizable.

The General Survey is a short 10 question survey that anyone who is a Surry County resident can take. It asks basic questions around health topics and resources.

Want to be able to voice your experiences in person and give as much detail as you need? Community Listening Sessions are in-person events that last about 2 hours. Facilitators will ask a few leading questions, but where the conversation goes is really up to you all! This helps us really understand the why behind health concerns and resources.
We have 13 total, one per elementary school district. See dates, times, and locations below:
As we find specific topics and problems we need to really deep-dive into, we will do a focus group. These are like community listening sessions, but they have a specific set of questions that we need answers for. Also, focus groups are invite-only instead of open to the general public. This is so we can get a specific part of the population together to provide insight, which limits extra factors from skewing answers.
As focus groups become available, we will post them in this section.
When we are looking for highly specific perspectives from a single individual, we will conduct one-on-one Key Interviews. This gives us in-depth perspective of a single individual who is representative of a broader discipline, community, organization, or population. These are generally limited to credible individuals who have the trust and respect of the group they represent.
These are not posted publicly, but it is another method that we use in the CHA to gather insight and data.
None of the work that we do would be possible without the help, support, and expertise of our community partners. We have 25 partners and counting! These agencies routinely contribute to bettering the health, wellbeing, and safety of Surry County.
The Surry County Health Foundation, as a Community Health Partner, hosts the public CHA page for the Surry County Health & Nutrition Center. It is just one of many ways that we support the mission of public health and the wellbeing of Surry County residents.
It's quiet work like this that makes a big impact on our county.
